This means that all events of a plot should be related. In a good story, othing happens that does not arise from preceding events. Only exception: the start. The first event in a book may be purely coincidental. But it starts all subsequent events - that is why it is called the triggering event. It's similar to the domino effect: the first stone falls and pushes over all the following ones. Of course, the best novels start with a bang!
